Repeat Online Prescription

Due to popular demand the Surgery has introduced an on line ordering service for repeat prescriptions. Please complete the request form at the bottom of this page.

You may use the service to order prescriptions that have been issued by a doctor and which you have been recommended to take regularly. The information you give us should be taken from the counterfoil slip of your prescription. We will need your full name and address and a contact phone number for any queries.

Please note we require 48 hours notice (excluding weekends and bank holidays) from point of ordering to collection of your prescription. Prescriptions requests are printed off for processing at 9.00am throughout the working week.

Any suggestions regarding this new service would be gratefully received.

Please type Date of Birth into the box shown below.
To order your repeat prescription please ensure you type in the name of each medication you require followed by the dose/strength (i.e. Aspirin 75mg)
