Repeat Online Prescription
Due to popular demand the Surgery has introduced an on line ordering service for repeat prescriptions. Please complete the request form at the bottom of this page.
You may use the service to order prescriptions that have been issued by a doctor and which you have been recommended to take regularly. The information you give us should be taken from the counterfoil slip of your prescription. We will need your full name and address and a contact phone number for any queries.
Please note we require 48 hours notice (excluding weekends and bank holidays) from point of ordering to collection of your prescription. Prescriptions requests are printed off for processing at 9.00am throughout the working week.

PLEASE NOTE: If you are ordering prescriptions for WARFARIN we will need to know your Date and result of your latest INR result - found in your Yellow Anticoagulant Book. Please type result in one of the medication slots. Thank you for your co-operation in this.
